I am a current or former customer of Lean for Good (the “Company”). As a result of my positive experiences with Company during the LeanBiome Challenge, I am willingly promoting the Company’s Services (defined below). Accordingly, I hereby grant to the Company, and each of the Company’s respective agents, advertisers, licensees, nominees, parents, subsidiaries, affiliates, successors and assigns and all others acting on behalf of each of the foregoing (collectively, the “Company Parties”), the right and permission to reproduce, copyright, depict, exhibit, transmit, broadcast, display, publish, distribute, communicate, visually identify and otherwise use my name, image, likeness, quotations, signature, biographical information (including my hobbies, occupation, interests and reasons for using the Company’s products and services), personal characteristics, performance, voice, portrait, photograph, signature, statements, opinions, testimonials, and/or any other indicia associated therewith and any recorded footage or photographs taken of me (collectively, my “Performance”) in connection with the advertising and promotion of the Company’s products and services (the “Services”), and any related publicity, business, trade or other purpose whatsoever, in perpetuity, throughout the world, in any language and in any and all media, whether now known or hereafter developed, including, but not limited to, broadcast media, print ads, brochures, television, radio, out-of-home media,
collateral media, electronic media, internet, digital media, mobile media, social media, interactive media, mobile applications, web ads, banner ads, promotional emails, and any derivatives of the foregoing. I further agree that my Performance may be edited by the Company Parties in any manner, and may be used alone, either in whole or in part, in character or form, in any and every form, style, size or color and/or with captions, film, video, artwork, sound effects, music audio track, photographs, or any other material, including without limitation, images and statements of others. I represent and warrant that all of my statements, opinions and/or testimonials, whether written or oral (collectively, “Statements”), provided by me to Company may be attributed to me, and are true and accurately reflect my honest opinion of Company and the Services. I further understand and agree that the results and proceeds (the “Results and Proceeds”) of my Performance and/or Statements and any other services that I may render to the Company Parties are specially ordered by Company and accordingly Company is deemed the author and exclusive owner thereof. If ownership of any portion of my Performance, Statements or the Results and Proceeds are not automatically vested in the name of the Company, I hereby perpetually, exclusively and throughout the world assign all right, title and interest in and to my Performance,
Statements and the Results and Proceeds to Company free and clear of any and all claims for royalties and other compensation. I further acknowledge and agree that no such uses of my Performance, Statements or the Results and Proceeds, including the use of such Performance, Statements or the Results and Proceeds in any advertisement or other material, need to be submitted to me for any review or approval and I hereby waive any right to review or approve any and all uses thereof.

I hereby release the Company Parties from any and all liability arising out of the aforementioned use of my Performance and/or Statements. I further agree to waive all rights and release and discharge the Company Parties from, and shall neither sue nor bring any proceeding against any such parties for any claim, demand or cause of action whether now known or unknown for defamation, right of publicity, or personality or any similar matter, or based upon or relating to the use of my Performance and/or Statements. I hereby waive any right that I may have to inspect and/or approve the use of my Performance and/or Statements or any advertising or copy that may be used in connection therewith, and/or any other use to which my Performance and/or Statements may be applied. My Statements may be used in whole or in part and may be paraphrased, amplified, shortened and/or put into conversational form to meet the requirements of copy, layout and/or script provided the general sense is not changed. I am executing this Testimonial and Image Release knowing that the Company Parties are relying on the accuracy and truthfulness of my Statements and Images and that the Company Parties will incur substantial expense to produce materials based upon my Statements and Images. I represent and warrant that this Testimonial and Image Release does not in any way conflict with any existing commitment on my part. Notwithstanding the foregoing, if my Performance and/or Statements are ultimately used by the Company in its marketing campaign for its Services, I shall not endorse or sell any third-party services competitive to the Services without first obtaining the Company’s prior written consent. I acknowledge that I am at least eighteen (18) years of age and has the full unrestricted right to execute this Testimonial and Image Release and grant the rights set forth herein. Lean for Good (the “Company”) reserves the right to reject any application received. In the event of this, the Company are not required to send the 6-month supply of LeanBiome, LeanBodyPro, and LeanSupreme, and in such circumstances agree not to use the testimonials/statements and images provided.
